Allegro Pediatrics in Bellevue, WA is seeking a compassionate Hybrid Behavioral Health Care Coordinator to support its integrated behavioral health program within the medical home. You will welcome referrals, guide families, coordinate intake and referrals, and help connect patients to community resources.

Responsibilities

• Welcome referrals and provide a steady, supportive point of contact for patients and families.
• Coordinate intake, scheduling, referrals, follow-up, and communication via phone/email.
• Maintain patient registries, track enrollment, and ensure timely documentation.
• Identify urgent behavioral health concerns and support triage through workflows.
• Guide families to community resources and support warm handoffs when available.
• Maintain referral resources and strengthen community partnerships.
• Contribute to quality improvement by identifying workflow gaps and refining the program.
