You will welcome referrals, coordinate intake and scheduling, guide families to community resources, and help maintain documentation, while upholding HIPAA and safety standards.

Qualifications

• High school diploma or GED required.
• Reliable transportation and ability to travel to multiple clinics.
• Healthcare or care coordination experience preferred but not required.

Responsibilities

• Welcome referrals and be a steady point of contact for patients and families.
• Coordinate intake, scheduling, referrals, and follow-up for integrated care.
• Maintain patient registries and timely documentation.
• Identify urgent concerns and support triage within workflows.
• Guide families to community behavioral health resources.
• Maintain referral resources and strengthen community partnerships.
• Support quality improvement by identifying workflow gaps.
