Job summary

Molina Healthcare is seeking a licensed Behavioral Health professional to serve as a Transition of Care Coordinator for Medicaid members recently discharged from psychiatric hospitalization. This hybrid role combines home office work with travel to local hospitals to ensure seamless service delivery and resources at discharge, ultimately reducing readmissions.

Applicants must reside in King or Snohomish County, hold WA licenses such as LICSW, LASW, LMHC, LMHCA, LMFT, ALICSW, ALASW, PhD or PsyD,

Qualifications

• Licensed behavioral health clinician with WA license(s) listed above.
• At least 2 years in health care with hospital discharge planning, care management or behavioral health.
• Valid driver’s license and reliable transportation for travel.
• Knowledge of Care Transitions Intervention (CTI) or similar model.

Responsibilities

• Follow members through a 30-day transition program from hospital to other settings to reduce readmissions.
• Collaborate with discharge planners, hospitalists, providers, facility staff and families.
• Ensure safe transitions with appropriate caregiving and medical oversight.
• Coordinate with ancillary providers and public agencies to arrange necessary services.
• Conduct face‑to‑face visits in hospital and at home for high‑risk members post‑discharge.
