Qualifications

• High school diploma or GED required.
• Experience navigating social service organizations.
• Lived, related, or professional experience with target population.
• Must pass background check and drug screen.

Responsabilités

• Coordinate outreach to families and engage children and youth in CHW support.
• Develop and maintain patient service and care plans.
• Assist with applications, insurance enrollment, and referrals.
• Provide health education and advocacy to families.
• Attend staff meetings and collect data for outcomes.
