Job summary

Headlight is seeking a Care Navigator to guide patients from behavioral health referrals to appropriate care. You will handle non-clinical navigation, reengage patients, and prevent referrals from falling through the cracks. You’ll work with partners onsite, virtually, or in a hybrid model, handling warm handoffs and escalation as needed.

The role emphasizes peer-informed care, patient-centered communication, and collaboration across partner networks to improve access and outcomes.

• Excellent verbal and written communication with patients and partners.

Responsibilities

• Serve as the initial point of contact with patients and healthcare partners.
• Receive real-time warm handoffs from clinicians and navigate to Headlight services.
