Job summary

Beth Israel Lahey Health, Inc. is seeking a Patient Navigation Specialist to provide coordinated, responsive support to patients requiring enhanced navigation across BILH and its community hospitals.

The hybrid role is based in Needham, MA with travel to other BILH locations as needed, three onsite days per week during the initial phase. The Specialist will work with clinical teams, administration, and philanthropy staff to expedite appointments, explain care pathways, and ensure equitable

Qualifications

• Bachelor’s degree preferred.
• HIPAA training required upon hire.
• 5–7 years of related experience in healthcare, patient services, care coordination, or a high touch service environment.
• Experience with Epic, Outlook, Word, Excel, PowerPoint; familiarity with philanthropy-based CRM.
• Strong interpersonal skills with the ability to build trust and communicate effectively across all levels of the organization.
• Experience with Microsoft 365 (Outlook, Word, Excel, and PowerPoint) is required.
• Prior familiarity with a philanthropy-based CRM and Epic is strongly preferred.
• Experience supporting patients with complex needs and/or navigating multi-site health systems, coordinating across departments or locations.

Responsibilities

• Serve as a primary resource for patients seeking assistance via phone, email, or in person interactions.
• Coordinate and expedite appointments, referrals, transfers, and access to specialty care across BILH and its community hospitals.
• Collaborate with the Divisional Vice President to support navigation needs across multiple sites.
• Promote communication between patients and care teams with dignity, respect, and equitable access.
• Maintain accurate documentation and manage patient navigation databases and reports.
