Beth Israel Lahey Health Performance Network is seeking a Population Health Navigator to assist primary care practices in improving care for patients with hypertension, diabetes, and those due for cancer screenings. This remote role involves outreach by phone, patient portal, and other channels, with occasional in-person meetings as needed.

Competenze

• High School diploma or GED required.
• 0–1 year of related work experience.
• Enrolled in a college-level program related to the field.
• Basic computer familiarity; able to navigate web-based applications.

Mansioni

• Assist with outreach and scheduling using phone, portal, and other modalities.
• Support pre-visit planning and reminders to improve patient care.
• Maintain patient registries and assist with data reporting.
• Provide effective communication between patients and care teams.
