Responsibilities

• Initiates communication with patients after a positive finding.
• Guides patients through the health care system and care continuum.
• Connects patients with community and social services.
• Liaisons between patient, PCP, surgeon and other providers.
• Facilitates communication among health care staff and providers.
• Provides health education to patients and staff.
• Maintains learning resources and patient education materials.
• Tracks interventions and outcomes, supports transitions of care.
