Qualifications

Responsibilities

• Performs direct outreach to patients to initiate engagement and enrollment with the clinic and schedule initial appointments.
• Gently follow up to overcome barriers to appointments or program completion and review after-visit summaries.
• Maintains KPI dashboards with the Medical Director on a recurring basis.
• Partners with a third-party vendor to ensure patients have devices and platform access, optimizing workflow for patients and clinicians.
• Uses Motivational Interviewing techniques to engage patients and provide emotional support throughout care.
• Documents patient communication in EMR using encounter notes, in-basket messages and MyChart.
• Verifies and updates patient insurance information when scheduling visits.
• Provides general clerical support: filing, scheduling, copies, mail, reminder calls, and contact database maintenance.
• Answers incoming clinic calls and escalates patient issues to the right team member.
• Contributes to workflow improvements and new ideas for the program.
