Beth Israel Lahey Health is seeking a Patient Navigation Specialist in Needham, MA. This hybrid role supports patients needing enhanced navigation across BILH and community hospitals, coordinating appointments, referrals, and access to specialty care with discretion and empathy.

The ideal candidate will have 5–7 years in healthcare or care coordination, strong communication, and experience with Epic and philanthropy CRM. On-site 3 days per week; travel to other locations as needed.

Qualifications

• Bachelor's degree preferred.
• 5–7 years of healthcare-related experience or care coordination.
• Experience with Epic, Outlook, Word, Excel, PowerPoint.
• Familiarity with philanthropy-based CRM systems.
• Strong interpersonal and written/verbal communication skills.

Responsibilities

• Serve as a primary resource for patients via phone, email, and in person interactions.
• Coordinate appointments, referrals, transfers, and access to care across BILH and community hospitals.
• Collaborate with senior leaders to support navigation across multiple sites.
• Explain care paths and resources to patients and families.
• Maintain accurate documentation and navigation databases; prepare reports as needed.
