Humana is seeking an Onsite Care Coordination Nurse to support members in acute care settings. You will perform face-to-face assessments and lead care coordination from hospital to home, addressing gaps in care and ensuring safe transitions.

The role combines in-person engagement at facilities with the flexibility to work remotely for coordination and documentation, in a hybrid work model. A meaningful, member-focused environment awaits.

Formación

• Current, unrestricted Registered Nurse license in Oklahoma.
• 3+ years acute care nursing experience.
• Experience with discharge planning or care coordination.
• Proficient in Microsoft Office and EMRs.

Responsabilidades

• Perform face-to-face assessments for newly admitted members.
• Lead discharge planning and post-discharge follow-up with providers.
• Identify barriers to discharge and coordinate care with teams.
• Educate members on diagnoses, medications, and preventive measures.
• Complete documentation accurately and timely.
