Job summary

Humana Inc. is hiring an Onsite Care Coordination Nurse to support members during transitions from hospital to home. This hybrid role combines in-person assessments at Humana facilities with remote coordination and documentation.

Responsibilities include face-to-face assessments, discharge planning, collaboration with care teams, and education to promote wellness and prevent readmissions. Requires Virginia RN license, 3+ years acute care, and 30–60 minute commute to VCU Medical Center in

Qualifications

• Current, unrestricted RN license in Virginia.
• Minimum of 3 years acute care nursing experience.
• Experience with discharge planning or care coordination.
• Experience in Microsoft Office applications and EMR.
• Valid driver’s license and proof of vehicle insurance.

Responsibilities

• Conduct comprehensive face-to-face assessments for newly admitted members at assigned acute care facilities.
• Lead discharge planning from admission through post-discharge.
• Identify gaps in care and develop strategies to address issues.
• Collaborate with providers, facility case managers, and other healthcare professionals.
• Educate on diagnoses, medications, and preventive measures to promote wellness and reduce readmission.
• Complete all required documentation accurately and promptly.
• Maintain infection control and patient privacy standards.
