Humana, Inc. seeks an Onsite Care Coordination Nurse in Kentucky to assess newly admitted members and lead safe discharge planning.

You will partner with hospital teams and PCPs to ensure timely transitions from hospital to home, addressing gaps in care and promoting member wellness. The role combines in-person care at an assigned facility with remote coordination, documentation, and scheduling flexibility within a hybrid work model.

Qualifikationen

• Current, unrestricted RN license in Kentucky.
• Must be located within a 30–60 minute driving distance to CommonSpirit- St Joseph Hospital, 1 St. Joseph Dr., Lexington.
• Minimum of 3 years of recent acute care nursing experience (medical-surgical, critical care, etc.).
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience in Microsoft Office applications and electronic medical records.
• Valid driver’s license, dependable automobile, and proof of vehicle liability insurance.

Aufgaben

• Conduct comprehensive face-to-face assessments of newly admitted members at acute care facilities.
• Support discharge planning from admission through post-discharge, including follow-up with PCPs.
• Identify gaps in care and barriers to discharge; develop and implement solutions.
• Collaborate with providers, case managers, and health professionals for coordinated care.
• Educate on diagnoses, medications, and preventive measures to reduce readmissions.
• Complete all required documentation accurately and promptly.
