Humana is seeking an Onsite Care Coordination Nurse to support members during critical moments in their care journey, coordinating care at acute facilities and facilitating safe hospital-to-home transitions. The role combines in-person assessments with remote coordination, within a hybrid work model at the Hillcrest Medical Center in Tulsa, OK.

The ideal candidate is a licensed RN in Oklahoma with at least 3 years of acute care experience, skilled in discharge planning and care coordination, and

Competenze

• Current, unrestricted RN license in Oklahoma.
• Must be located within a 30-60 minute driving distance to the Hillcrest Medical Center (Tulsa, OK).
• Minimum of 3 years of recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience in Microsoft Office applications and electronic medical records.
• Valid driver’s license, dependable vehicle, and proof of insurance.

Mansioni

• Conduct comprehensive, face-to-face assessments of newly admitted members at acute care facilities to evaluate needs.
• Support and educate members on discharge planning from admission through post-discharge.
• Identify gaps in care and barriers to discharge; develop strategies to address issues.
• Collaborate with providers and care teams for timely, coordinated discharge planning.
• Provide education on diagnoses, medications, and preventive measures to promote wellness and reduce readmission risk.
• Complete all required documentation accurately and promptly.
• Maintain adherence to infection control and patient privacy standards.
