Job summary

Humana is seeking an Onsite Care Coordination Nurse to support members at acute care facilities in Tulsa, OK. You will conduct comprehensive assessments, lead discharge planning, and collaborate with care teams to ensure safe transitions from hospital to home.

Qualifications

• RN license in Oklahoma.
• Must live within a 30–60 minute drive to Hillcrest Medical Center in Tulsa.
• Minimum 3 years of acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience in Microsoft Office and electronic medical records (EMR).

Responsibilities

• Conduct face-to-face assessments for newly admitted members at acute care facilities.
• Lead discharge planning from admission through post-discharge.
• Identify gaps in care and develop strategies to address them.
• Collaborate with providers, facility case managers, and care teams for coordinated care.
• Educate members on diagnoses, medications, and preventive measures.
• Complete documentation accurately and promptly.
• Maintain adherence to infection control and privacy standards.
