Qualifications

• Active, unrestricted RN license in Oklahoma.
• Minimum of 3 years recent acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience with Microsoft Office applications and electronic medical records.
• Valid driver’s license and dependable vehicle with insurance.

Responsibilities

• Conduct face-to-face assessments for newly admitted members at acute care facilities.
• Lead discharge planning and follow-up visits with providers.
• Identify gaps in care and coordinate with care teams to address them.
• Collaborate with attending providers and facility case managers for timely discharge planning.
• Educate members on diagnoses, medications, and preventive measures.
• Complete documentation accurately and promptly.
• Maintain infection control and privacy standards.
