Qualifications

• Current, unrestricted RN license in Oklahoma.
• Minimum 3 years acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Proficient in Microsoft Office and EMR systems.

Responsibilities

• Conduct face-to-face assessments for newly admitted members at acute care facilities.
• Lead discharge planning from admission through post-discharge; arrange follow-ups.
• Identify gaps in care and implement strategies to address them.
• Collaborate with providers and care teams for timely, coordinated discharge.
• Educate members on diagnoses, medications, and preventive measures.
• Complete documentation accurately and promptly.
• Maintain infection control and patient privacy standards at all times.
