Job summary

Humana is seeking an Onsite Care Coordination Nurse to support members at acute care facilities with face-to-face assessments and active care coordination. The role emphasizes safe transitions from hospital to home, collaboration with care teams, and education for patients to promote health and prevent readmissions.

The position combines onsite facility work with remote coordination and documentation, offering a hybrid schedule and the possibility to travel to Humana offices as needed.

Qualifications

• Current, unrestricted RN license in Oklahoma is required.
• Must reside within 30–60 minutes driving distance to the Hillcrest Medical Center.
• Minimum 3 years of acute care nursing experience.
• Experience with discharge planning or care coordination in hospital or health plan settings.
• Experience using Microsoft Office and EMR systems.
• Valid driver’s license and personal vehicle insurance.

Responsibilities

• Perform face-to-face assessments for newly admitted members.
• Lead discharge planning from admission through post-discharge.
• Identify gaps in care and address barriers to discharge.
• Collaborate with providers and care teams for coordinated care.
• Educate members on diagnoses, medications and preventive measures.
• Complete documentation accurately and maintain privacy standards.
