Job summary

Great Lakes Integrated Network in Buffalo, NY seeks an Oncology Nurse Navigator to guide patients through diagnosis, treatment and survivorship, coordinating education, support, and care across the oncology team.

Hybrid role requiring an RN license, 2+ years of oncology experience, and ability to connect patients with community resources; emphasizes shared decision-making, cultural sensitivity, and timely access to services throughout the cancer journey.

Qualifications

• Associate’s degree in Nursing is required and must be from a program accredited by CCNE/ACEN/CNEA
• BSN degree preferred
• RN license required; valid driver’s license; OCN or AONN certification preferred; obtain AONN certification within 2 years

Responsibilities

• Collaborate with the oncology team to coordinate services across the cancer continuum
• Educate patient and family about diagnosis, treatments, symptoms, trials, and support services
• Assess patient and family needs that could interfere with treatment
• Connect patients with community resources and support services
• Complete SDoH assessment to identify barriers and connect to resources
• Explain cancer care issues in the patient’s language and use interpreter services as needed
• Participate in quality and site meetings and grant process improvement initiatives
• Identify barriers to care and recommend strategies to address them
• Develop navigation assessment tools, guidelines, pathways and algorithms
• Coordinate multidisciplinary cancer conferences and document recommendations
• Complete annual cancer-focused community needs assessment
• Maintain confidentiality and participate in patient advisory groups
• Complete required oncology navigation education hours annually
• Perform other duties as assigned
