Job summary

Great Lakes Integrated Network in Buffalo, NY, seeks an Oncology Nurse Navigator to coordinate cancer care across the continuum in a hybrid role with some travel.

The position requires RN licensure, 2+ years of oncology experience, and strong communication, education, and care coordination skills. Benefits include health, dental, 401K, vacation, and PTO.

Salary ranges from $75,000 to $85,000 annually and will be determined by experience and market data.

Qualifications

• Active RN license
• Minimum two years oncology nursing experience
• Experience in high-volume, fast-paced setting
• Oncology ambulatory nursing care coordination experience preferred

Responsibilities

• Coordinate appointments, tests, and procedures with the oncology team
• Educate patients and families about diagnosis, treatments, and support services
• Assess patient and family needs that could interfere with treatment
• Connect patients to internal and external resources
• Complete SDoH assessments and link to resources
• Explain cancer care issues in plain language and use interpreters as needed
• Review local support groups and education addressing cultural issues
• Maintain confidentiality of patients, families, staff
• Participate in necessary quality, site, and grant meetings
• Develop navigation assessment tools and care pathways
• Participate in cancer conference coordination and documentation
