Job summary

Lantern is seeking an Oncology Nurse Navigator to partner with members navigating a cancer diagnosis, providing education, care coordination, and ongoing support throughout treatment and survivorship. This hybrid role is based in Dallas, TX or New York, NY with a base salary range of $90,000–$105,000 plus bonus.

Requirements include a BSN, active RN license, ONCC/CPHON/CBCN certification, and familiarity with NCCN guidelines.

Qualifications

• Bachelor's degree in Nursing (BSN).
• Active RN license in a compact state and able to be licensed in all 50 states.
• Minimum 3 years oncology nursing experience.
• Active ONCC certification (OCN, CPHON, or CBCN) with ability to maintain.
• Proficiency in Outlook and Office Suite.
• Familiarity with NCCN guidelines and ClinicalTrials.gov.
• Hybrid role; Dallas or New York area-based.

Responsibilities

• Manage a full, active caseload of cancer-diagnosed members with education and clinical support through survivorship.
• Conduct oncology intake assessments to establish member needs, goals, and care priorities.
• Educate members and families on diagnosis, treatment options, and resources using NCCN/ONCC guidance.
• Coordinate multidisciplinary care by facilitating communication among members, providers, and Lantern team.
• Identify and address barriers to care including psychosocial, financial, and logistical needs.
• Document all interactions and clinical activities within Lantern's care management platform.
• Complete outreach, follow-ups, and care coordination tasks within defined timeframes.
• Participate in call auditing and apply feedback to improve quality of care.
• Collaborate with Lead ONN, LCSWs, Medical Records, Care Advocates, and partners.
• Contribute to team meetings, case reviews, and quality improvement initiatives.
• Adhere to Lantern workflows, protocols, and documentation standards.
