Job summary

Central Health is seeking a Nurse Navigator to support patients through complex healthcare environments, coordinating appointments and treatments. The role works autonomously and with the care team to remove barriers to care and optimize outcomes, while modeling the organization’s values and commitment to exceptional patient experience.

This hybrid position may require work at an approved offsite location and onsite at primary locations, depending on business needs.

Qualifications

• RN license is required and must be active in Texas.
• Experience with ambulatory clinics or hospital settings preferred.
• Knowledge of Joint Commission standards and HIPAA regulations is desirable.

Responsibilities

• Collaborate with patient care teams to optimize clinical outcomes.
• Coordinate scheduling of appointments, diagnostics and procedures.
• Track referrals, interventions and outcomes to ensure continuity of care.
• Educate patients and families about diagnosis, treatments and medications.
• Use AIDET and motivational interviewing techniques to support patient self-management.
• Assist in development of departmental protocols and quality improvement initiatives.
