Job summary

World Class Health is seeking a Nurse Navigator (RN) for a hybrid role to coordinate end-to-end surgical episodes for employees and dependents traveling to Centers of Excellence worldwide. You will deliver remote case management across pre- and post-operative phases and ensure safe transitions of care.

The role requires active RN license, 3–5+ years of clinical experience, and strong coordination across providers.

Qualifications

• RN with active license and 3–5+ years clinical experience.
• 2+ years of case management experience.
• 1+ year of startup experience.
• 3+ years of remote working experience.
• Able to be in SF office up to 6 days a month.
• Tech savvy; proficiency with Google Workspace and AI tools.
• Experience coordinating across COE networks and multiple providers.
• CCM certification is preferred.

Responsibilities

• Coordinate end-to-end surgical episodes for employees and dependents.
• Deliver remote case management from pre-op to post-op.
• Guide COE selection, scheduling, pre-op readiness, and travel logistics.
• Monitor travel and hospital admission; maintain stakeholder updates.
• Support adherence to care plans and post-op recovery; manage medications and rehab guidance.
• Educate patients to improve activation and self-care.
• Ensure timely communication among COE teams, PCPs, and plan stakeholders.
• Track outcomes and drive continuous improvement in patient experience.
• Assess medical, social, and functional needs during transitions of care.
