Central Health in Austin is seeking a Nurse Navigator to guide patients through complex health systems from our Navigation Center. This role operates autonomously and in collaboration with the care team, providing inbound and outbound call support to improve access to care and timelines for appointments.

You will coordinate referrals, track outcomes, educate patients on diagnoses and medications, and support transitions of care with our care management teams.

Qualificações

• Graduation from an accredited school of nursing with at least an Associate's Degree.
• Two years of nursing experience in ambulatory clinic or hospital setting.
• BSN preferred; navigation or case management experience is a plus.

Responsabilidades

• Collaborates with the patient’s care team to optimize outcomes in a call center/hybrid setting.
• Coordinates scheduling of appointments and referrals; tracks outcomes.
• Educates patients and families on diagnosis, treatments, procedures, and medications via call center.
• Supports transitions of care with care management teams to assist patients with complex needs.
• Enhances patient experience by managing provider in-basket tasks and medication refills.
• Participates in quality improvement projects to improve service quality.
• Attends staff meetings and supports a positive workplace culture.
