Job summary

CP Unlimited seeks an Intake Patient Navigator to support its Health Home Care Management program. This role serves as the initial contact for prospective members, helping them understand services and navigate referrals, eligibility, intake, and enrollment.

Qualifications

• Bachelor’s degree preferred; an associate’s degree with relevant experience may be considered.
• 1–3 years of intake, patient navigation, outreach, healthcare, case management support, or human services experience.
• Strong customer service, communication, and interpersonal skills.

Responsibilities

• Receive and process referrals from healthcare providers and community sources.
• Explain Health Home care management services and assist with the intake process.
• Conduct initial screenings for medical, behavioral health, and social service needs.
• Review Medicaid and program eligibility information per procedures.
• Assist with enrollment forms, consents, and required documentation.

Education

Bachelor’s degree preferred; associate’s degree with relevant experience considered
