Job summary

Minnewaska Community Health Services is seeking an experienced MDS Coordinator (RN) to join our team in Starbuck, MN. The role focuses on timely assessments, interdisciplinary care plans, and accurate MDS coding to meet Federal and State guidelines.

Qualified candidates have 3+ years of MDS experience, Minnesota RN licensure, and demonstrated ability to educate staff while maintaining CEU requirements and background checks. On-site work with occasional travel is required.

Qualifications

• Must have current/active RN license with the State of Minnesota and 3 or more years of MDS experience.
• Must keep up with CEU’s for license renewal.
• Must have the ability to communicate and comprehend in order to gather information from residents and their families.
• Must be able to educate and train employees with diverse ages and backgrounds on the correct way to code resident’s ADLs.
• The MDS Coordinator is also required to have an ability to read, write and speak English, and complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.

Responsibilities

• Coordinate timely completion of assessments and care plans.
• Identify resident acuity and determine specific care needs and communicate to responsible parties.
• Work with interdisciplinary care teams to ensure accurate MDS coding and documentation.
• Provide education and guidance to staff on regulatory requirements and best practices.
