Job summary

Minnewaska Community Health Services is seeking an experienced MDS Coordinator (RN) to ensure timely, accurate assessments and care plans in compliance with federal and state guidelines. You will identify resident acuity, determine care needs, and communicate with families and interdisciplinary teams.

The role requires an active Minnesota RN license, 3+ years of MDS experience, ongoing CEU compliance, and strong English communication skills.

Qualifications

• Active RN license in Minnesota required.
• 3+ years of MDS experience.
• Maintain CEUs for license renewal.
• Ability to gather information from residents and families through clear communication.
• Ability to educate and train staff on coding ADLs and MDS processes.
• Must read, write and speak English and pass a state background check.

Responsibilities

• Coordinate timely assessments, interdisciplinary care plans and MDS documentation.
• Identify resident acuity and needs, and communicate requirements to the care team.
• Educate and support staff on proper ADL coding and regulatory requirements.
