Qualifications

• Graduation from an accredited LVN nursing program.
• Current valid LVN license to practice.
• Experience in a multi-disciplinary team.
• Three years in public health nursing, acute care, case management, or home health; 2 years managed care preferred.
• Bilingual in English and Spanish preferred.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Provide daily care coordination with concurrent review for hospital/SNF patients.
• Perform case management for admitted hospital/SNF patients and ambulatory enrollment.
• Identify CCS cases, manage transfers and retro reviews.
• Collaborate with social work, ambulatory case management, and medical staff.
