Job summary

Proven LVN licensure in California and BLS are required. Candidates with experience in MSO settings, Medicare Advantage, Medi-Cal populations, and EMR care management systems are preferred.

Qualifications

• Minimum: High School diploma or equivalent; graduation from an accredited LVN program.
• Preferred: Additional training in case management, utilization management, or care coordination.
• Minimum: At least two years of clinical LVN experience.
• Preferred: Experience in concurrent review, inpatient utilization management, MSO/IPA/health plan.
• Current California LVN license and BLS certification.

Responsibilities

• Support inpatient and post-acute care coordination and concurrent review activities.
• Monitor inpatient and post-acute utilization and assist discharge planning.
• Facilitate transitions of care and coordinate services across care continuum.
• Collaborate with hospitals, SNFs, IRFs, LTACHs, home health, physicians, and care teams.
