AltaMed Health Services Corp. is seeking a Licensed Vocational Nurse (LVN) Case Manager in Commerce, CA. The role focuses on daily care coordination, with concurrent review for patients in hospital or SNF, and extends to ambulatory case management.

Collaboration with social work and physicians is essential. The successful candidate will have LVN licensure, multi-disciplinary teamwork experience, and a willingness to support diverse patient populations, with bilingual English/Spanish preferred

Qualifications

• Graduation from an accredited LVN nursing program.
• Current valid License as a Licensed Vocational Nurse.
• Experience in a multi-disciplinary team.
• Three years of public health nursing, acute care or case management preferred.
• Bilingual in English and Spanish preferred.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Provide daily care coordination with concurrent review for hospital/SNF patients.
• Case manage patients admitted to hospital or SNF and those enrolled in ambulatory care.
• Identify CCS cases, manage transfers and retro reviews.
• Collaborate with social work, ambulatory case management, and physicians.
