AltaMed Health Services Corp. invites applications for a Licensed Vocational Nurse, In-Patient Case Manager.

The role provides daily care coordination with concurrent review for patients in hospital or SNF, including telephonic support, and involves case management for admissions and ambulatory care. As part of an interdisciplinary team, you will coordinate with social work, ambulatory case management, hospitalists, and physicians, identify CCS cases, and support transfers and retro reviews;

Qualifications

• Graduation from an accredited LVN nursing program.
• Current valid License as a Licensed Vocational Nurse.
• Experience in and willingness to be part of a multi-disciplinary team.
• Three years' experience in public health nursing, acute care, case management, and/or in-home health care.
• Minimum of 2 years of managed care experience in case management with inpatient/outpatient focus.
• Bilingual in English and Spanish preferred.
• A valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Provide daily care coordination with concurrent review for patients in hospital or SNF, telephonically.
• Coordinate case management for patients admitted to hospital or SNF and those enrolled in ambulatory case management.
• Identify California CCS cases and handle transfers and retro reviews.
• Collaborate with social work, ambulatory case management, hospitalists, physicians, and nursing staff.
