Job summary

AltaMed is seeking an LVN Case Manager to provide daily care coordination and concurrent review for patients in hospital or SNF, including enrollment in ambulatory case management. You will identify CCS cases, manage transfers, and support retro reviews as part of an interdisciplinary team with social work, ambulatory care, and physicians.

The role requires graduation from an accredited LVN program, an active LVN license, and multi-disciplinary team experience.

Qualifications

• Graduation from an accredited LVN nursing program.
• Current valid LVN license to practice.
• Experience in multi-disciplinary teams and case management.

Responsibilities

• Provide daily care coordination with concurrent review for hospital/SNF patients (telephonically).
• Coordinate with social work, ambulatory care management, and physicians.
• Identify CCS cases, handle transfers, and perform retro reviews.
