Job summary

AltaMed Health Services Corp. is seeking an LVN Case Manager to deliver daily care coordination for patients in hospitals or SNFs, with telephonic reviews and ambulatory follow-up in California.

You will collaborate with social work, ambulatory case management, regional medical directors, hospitalists and clinic physicians to ensure seamless transitions and CCS case identification. A valid LVN license and bilingual English/Spanish are preferred for this role.

Qualifications

• Graduation from an accredited LVN nursing program.
• Current valid License as a Licensed Vocational Nurse.
• Experience in and willingness to be part of a multi-disciplinary team.
• Three years’ experience in public health nursing, acute care, case management, and/or in-home health care; minimum 2 years managed care experience in case management, inpatient/outpatient.
• Bilingual in English and Spanish preferred.
• A valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Provide daily care coordination with concurrent review for patients in hospital or SNF, including telephonic contact.
• Coordinate with social work, ambulatory case management, Regional Medical Directors, Hospitalists and clinic physicians to ensure integrated care.
• Identify CCS cases, manage transfers, and support enrollment in ambulatory case management.
