Job summary

AltaMed Health Services Corporation is seeking a Licensed Vocational Nurse (LVN) Case Manager for a hybrid role in Commerce, CA. You will coordinate daily care for patients in hospital or SNF settings, including telephonic reviews and ambulatory enrollment when appropriate.

The ideal candidate has an LVN license, graduation from an accredited LVN program, 3 years of relevant experience, and a BLS certification. Bilingual English/Spanish is preferred to support diverse communities.

Qualifications

• Graduation from an accredited LVN nursing program.
• Current valid LVN license.
• Three years’ experience in public health nursing, acute care, case management, and/or in-home health care.
• Bilingual English and Spanish preferred.
• A valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Coordinate daily care for hospital or SNF patients, including telephonic reviews.
• Provide inpatient and ambulatory case management and transfers.
• Identify CCS cases and perform retro reviews as needed.
• Collaborate with social work, ambulatory teams, and hospital staff to ensure seamless care.
