Job summary

Centene is seeking a Care Manager to support aging and disability waivers in Illinois. This hybrid-remote role requires travel to member homes and involves coordinating with care teams to develop and monitor individualized plans.

Responsibilities include evaluating needs, identifying resources, and educating members and families on benefits and procedures. Training runs 8–5 CT initially, with subsequent hours 8–4:30 CT.

Qualifications

• Bachelor's degree and 1 year of related experience required.
• RN licensed in Illinois qualifies under plan requirements.
• LPN with one year experience in comprehensive assessments qualifies.
• Bachelor or Master’s in human services may meet the educational criteria.

Responsibilities

• Evaluates member needs and resources and recommends the plan for best outcomes.
• Develops long-term care plans and identifies providers and community resources.
• Coordinates between member/family and care team to ensure service access.
• Provides resource support for employment, housing, and independent living needs.
• Monitors plans, member status, and outcomes with recommendations.
• Interacts with long-term care providers and partners to meet needs.
• Maintains and documents member information to ensure regulatory compliance.
• May perform on-site visits to assess needs and coordinate with providers.
• Educates members and families on procedures, services, and benefits.
• Provides feedback to leadership to improve quality and cost-effectiveness.
• Performs other duties as assigned; complies with policies and standards.
