Job summary

Elevance Health is seeking a LTSS Service Coordinator – Clinician to work in the field across East/Middle/West Tennessee. The role emphasizes field-based engagement, with occasional office attendance, and adherence to hybrid-work policies.

You will identify high-risk members, collect clinical data, and collaborate with RNs to develop and adjust care plans for individuals with chronic illnesses or disabilities.

Qualifications

• Requires LPN/LVN, LSW, LCSW, LMSW or RN license per state law
• Minimum 2 years of experience with chronic illnesses or disabilities in a service coordination or similar role
• Current, unrestricted LPN/LVN, LSW, LCSW, LMSW or license as allowed by state law
• BA/BS in Health or Nursing preferred

Responsibilities

• Identify members at high risk and assist in care planning.
• Obtain clinical data as directed.
• Identify candidates for alternative care or waiver programs.
• Provide collected info to RN for care plan development and monitoring.
• Coordinate care for members with chronic conditions with the health care team.
• Apply clinical guidelines in decision making, without nursing judgment.
