Qualifications

• Bachelor’s degree with 2+ years of relevant experience, OR
• RN license with 2+ years of relevant experience, OR
• Master’s degree with 1+ year of relevant experience.

Responsibilities

• Advocate for and with members to ensure informed decision making and appropriate guardianship.
• Lead and collaborate with the interdisciplinary team to develop Life Plans.
• Implement and monitor Life Plans and conduct reviews and approvals.
• Coordinate supports across medical, behavioral, social, habilitation, and community services.
• Maintain contact with key people in a member’s life.
• Ensure timely submission of Life Plans and progress notes.
• Assist members with benefits such as Social Security, SSI, Medicaid/Medicare, and Food Stamps.
• Help members resolve housing, utilities, safety, and judicial system issues.
• Report abuse or neglect immediately when observed or reported.
