Geode Health in Athens, GA seeks a fully licensed Mental Health Therapist to join our hybrid care model. You will diagnose and treat mental health disorders and provide psychotherapy across the spectrum, collaborating with an integrated care team and a prescriber as needed.

Qualifications

• Licensed Therapist in the state of Georgia (LCSW/LMSW/LSW/LPC/CPCS/LMFT or PsyD) with commitment to full licensure.
• Proficiency with electronic medical records (EMR) and up-to-date documentation.
• Understanding of mental health disorders and treatment modalities.
• Ability to provide care across a general spectrum of mental health conditions.

Responsabilités

• Diagnose and treat mental health disorders and provide psychotherapy.
• Collaborate with integrated care team and prescriber for coordinated care.
• Provide crisis intervention when needed.
• Develop personalized treatment plans and monitor progress.
• Support care coordination and patient follow-up.
