Job summary

Geode Health is seeking a Licensed Mental Health Therapist to diagnose and treat mental health disorders and deliver psychotherapy across the general spectrum of conditions. You will work within an integrated care team to coordinate care, including crisis interventions when needed.

Our model uses evidence-based care and measurement-based outcomes to track progress. The role offers a hybrid schedule with three days onsite and two days via telehealth, plus competitive compensation and benefits.

Qualifications

• Active license (LCSW, LCPC or LMFT) in the state of Illinois.
• Experience conducting comprehensive assessments and developing treatment plans.
• Proficiency in using an EMR system and ensuring up-to-date patient records.
• Commitment to evidence-based mental health care.
• Desire to treat a general spectrum of conditions.
• 1+ year of psychotherapy experience preferred.

Responsibilities

• Diagnose and treat mental health disorders and provide psychotherapy.
• Collaborate with an integrated care team for care coordination.
