Job summary

Geode Health is seeking a Licensed Mental Health Therapist to diagnose and treat mental health disorders and deliver psychotherapy across a broad spectrum of conditions. You will collaborate with a care team to coordinate services and provide crisis intervention as needed.

The role supports a hybrid work model with three onsite days and two telehealth days, enabling flexible scheduling and ongoing professional development. A strong focus on evidence-based care guides daily practice.

Qualifications

• Active Illinois license (LCSW, LCPC or LMFT).
• Experience conducting comprehensive assessments and developing individualized treatment plans.
• Proficiency with EMR systems and maintaining accurate records.
• Ability to work across a broad spectrum of mental health conditions.
• At least 1 year of psychotherapy experience.

Responsibilities

• Diagnose and treat mental health disorders and provide psychotherapy to clients.
• Collaborate with an integrated care team to coordinate services.
• Offer crisis intervention when needed and ensure patient safety.
• Partner with providers to advance patient goals and treatment plans.
