Job summary

Qualifications

• At least 3 years in human services.
• Strong recovery support competencies and ethics.
• Must have strong written and verbal communication and computer literacy.
• Bachelor’s degree required; Social Work or Human Services preferred.

Responsibilities

• Support residents in transitioning to independent living.
• Meet residents three times weekly to monitor housing stability.
• Engage residents with a trauma-informed, culturally humble approach.
• Document care and coordinate TennCare authorizations for continued enrollment.
• Collaborate with the Permanent Supportive Housing team and community partners.
• Attend staff meetings and participate in on-call rotations.
