Job summary

Project HOME seeks a Residential Services Coordinator II to support families in a family‑centered housing program. You will perform case management, connect residents to housing, income, education, and health services, and help families achieve stability and self‑sufficiency.

The role requires strong crisis intervention skills and compassionate, person‑centered care. Responsibilities include coordinating resources, developing recovery plans, and maintaining accurate documentation.

Qualifications

• Bachelor's degree or higher required or equivalent experience.
• Minimum two years of direct case management experience.
• Experience with homelessness or behavioral health populations preferred.
• Availability for evenings, weekends, and on-call rotation.

Responsibilities

• Build trusting relationships with residents and support transition from homelessness to stable housing.
• Conduct assessments to identify strengths, needs, barriers, and goals.
• Provide ongoing case management including goal planning, advocacy, and follow-up.
