Qualifications

• Bachelor’s degree required; Social Work or Human Services preferred or equivalent practical experience.
• 3+ years of experience in human services.
• Valid Tennessee Driver’s License and proof of personal automobile insurance.
• Strong written and verbal communication skills; computer literacy.

Responsibilities

• Assist residents as they transition from a supported housing environment to independent living.
• Meet with members three times a week to maintain housing stability.
• Provide trauma-informed, recovery-oriented tenancy support.
• Develop and submit TennCare authorizations for continued enrollment in services.
• Maintain documentation for agency and funding requirements.
• Collaborate with staff and partners to coordinate care across services.
