Job summary

The Columbus Organization in Indiana seeks a Case Manager for a hybrid role coordinating services for individuals with intellectual and developmental disabilities. You will ensure access to waivers and community supports, while advocating for meaningful outcomes and maintaining regulatory compliance.

The role requires collaboration with families, guardians, providers, and state agencies, plus travel within assigned counties and adherence to PCISPs and documentation standards.

Qualifications

• Must meet one of the listed educational/experience paths.
• Bachelor’s degree in social services-related field or equivalent plus relevant experience; or RN with human services experience; or four years of related experience with at least one year in IDD.
• Demonstrated initiative, accountability, and ability to work independently.
• Strong interpersonal and relationship-building skills.
• Commitment to person-centered practices and advocacy.
• Ability to travel for in-home visits, meetings, and community work.
• Must maintain automobile insurance and have access to a vehicle.
• Must reside within the assigned service territory.

Responsibilities

• Build and maintain relationships with participants, families, guardians, providers and agencies.
• Ensure HIPAA compliance and confidentiality.
• Develop person-centered plans and PCISPs.
• Conduct annual planning and assessments including LOCSI and budgets.
• Lead IST meetings at least semi-annually.
• Perform required face-to-face visits including annual in-home visits.
• Maintain engagement with participants at least every 90 days.
• Monitor service delivery and utilization.
• Promote health, safety and risk mitigation.
• Evaluate outcomes and satisfaction to drive improvements.
• Maintain monthly case notes and timely documentation.
• Complete monitoring and compliance activities and follow-ups.
• Manage incident reporting per BQIS and communicate with stakeholders.
• Disseminate PCISPs, authorizations, notices and incident reports.
• Support resource navigation and community integration.
