Job summary

Indiana Professional Management Group, Inc (IPMG) is hiring a Case Manager to serve Marion County with a hybrid work arrangement. The role requires travel within the county and surrounding areas for community meetings several times a week, as in-person attendance is sometimes required.

Applicants must reside within Marion County, IN. You will educate individuals about Medicaid waivers, develop individualized support plans, help them access needed services, and coordinate with a team to review

Qualifications

• Bachelor's degree in a human services field or equivalent is required.
• Direct experience with individuals with intellectual/developmental disabilities or aging population is required.
• Must possess a valid driver’s license and reliable transportation.

Responsibilities

• Provide information and education to help individuals understand the Medicaid waiver process.
• Create an Individual Support Plan based on the strengths, needs, goals and desires of the individuals we serve.
• Help individuals access services and supports they need to meet their goals.
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed.

Education

Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation or related human services field

Bachelor's degree in an unrelated field plus 1 year direct experience working with people with intellectual/developmental disabilities or 2 years aging population experience

Valid driver's license, car insurance, and reliable transportation
