Job summary

IPMG is seeking a Case Manager for a full-time hybrid role serving Hendricks County, Indiana. The position requires residing in Hendricks County and occasional travel across the county for in-person meetings with community partners.

You will provide Medicaid waiver information, develop Individual Support Plans, help clients access services, and coordinate care through regular team meetings with providers.

Qualifications

• A Bachelor's degree in psychology, sociology, social work, special education, nursing, gerontology, counseling, rehabilitation, or another human services field, or
• A Bachelor's degree in an unrelated field and a minimum of one year of direct experience working with people with intellectual and/or developmental disabilities, or two years experience working with the aging population
• A valid driver's license, car insurance, and reliable transportation
• The ability to pass thorough background screens
• Designated home office setting with computer, internet service, and smart phone

Responsibilities

• Provide information and education to help Individuals understand the Medicaid waiver process
• Create an Individual support plan based on the strengths, needs, goals, and desires of the Individuals we serve
• Help Individuals access services and supports they need to meet their goals
• Facilitate regular community-based team meetings to review plan of care and collaborate with service providers as needed

Education
