Job summary

Abode Services is seeking a Service Coordinator to deliver housing services and coordinate supports for individuals experiencing homelessness. You will develop individualized housing plans, work with landlords and providers, and help participants achieve housing stability.

The role requires experience in case management, a valid CA driver's license, and the ability to meet clients in the community or at home as needed. Onsite position with potential for hybrid/remote work per supervisor approval.

Qualifications

• Experience working with homeless populations.
• 2 years case management experience providing services to homeless or low-income individuals and/or families.
• Bachelor's degree in Sociology, Human Services or related field, or equivalent experience.
• Valid California Driver's License and proof of auto insurance with clean DMV record.

Responsibilities

• Provide housing services to participants to increase housing stability.
• Coordinate services with landlords and community partners to ensure access to essential supports.
• Develop individualized housing plans including pathways to permanent housing stability.
• Meet households in person in the community or home as needed.
• Maintain client files and timely documentation, using BIRP format.
• Participate in program meetings and coordinate with internal/external partners.
