Job summary

Abode Services is seeking a Service Coordinator to deliver housing services to individuals experiencing homelessness. You will develop individualized housing plans, coordinate referrals, and connect participants with financial, legal, and supportive services to advance housing stability.

Responsibilities include working with landlords, collaborating with partner agencies, maintaining accurate records, and meeting participants where they are to create pathways to permanent housing.

Qualifications

• Bachelor's degree in Sociology or related field.
• 2 years case management experience with homeless or low-income clients.
• Valid California driver’s license with current auto insurance.
• Flexible schedule including evenings and weekends.

Responsibilities

• Provide comprehensive housing services and individualized housing plans.
• Coordinate referrals to financial, credit counseling, legal services and other supportive services.
• Work with landlords and partners to secure housing and ensure stability.
• Maintain client files and document services in a timely manner.
• Attend meetings and communicate with team to coordinate services and improve outcomes.
