Job summary

Abode is seeking a Service Coordinator to deliver housing services to individuals experiencing homelessness, including counseling, referrals, and individualized housing plans. The role focuses on coordinating with landlords and partners to enhance housing stability and access to supportive services.

Applicants should have a Bachelor's degree in a related field and at least 2 years of case management experience.

Qualifications

Bachelor’s degree in Psychology, Human Services, Social Work, Sociology, or related field.
2 years case management experience providing services to homeless or low-income individuals and/or families.
Driving and transportation of participants when required.
Use of personal vehicle and proof of valid California Driver's License and current auto insurance, along with a clean DMV record, is required.
Flexible schedule to work evening and weekend hours as needed.

Responsibilities

Provide comprehensive housing services to participants, including working directly with landlords.
Coordinate services with program partners to ensure essential services are delivered.
Develop and maintain individualized service plans and needs assessments for each household.
Meet with households in person and attend program meetings to coordinate services and improve outcomes.
