Abode Services is seeking a Service Coordinator in Watsonville, CA, to support individuals experiencing homelessness by developing housing plans and coordinating access to counseling, financial services, legal aid, and supportive housing.

You will work with landlords and community partners to maximize housing stability, conduct in-person visits, and maintain thorough case notes. A CA driver’s license, flexible schedule, and strong communication skills are essential to success.

Qualificações

• Bachelor's degree in Psychology, Human Services, Social Work, Sociology, or related field or equivalent field experience.
• 2 years case management experience providing services to homeless or low-income individuals and/or families.
• Driving and transportation of participants when required; valid CA Driver's License and auto insurance with clean DMV record.
• Flexible schedule including evenings and weekend hours as needed.

Responsabilidades

• Provide housing services to homeless participants, including counseling and referrals to financial/credit, legal services, and individualized housing plans.
• Coordinate services to connect participants with essential supports and ensure health and housing outcomes.
• Meet with households in person, in the community or at home, as frequently as needed.
• Attend program meetings with internal and external partners to coordinate services and share resources.
• Maintain client files and document services promptly, using BIRP format.
